Follow this design checklist to help you design a survey that will provide actionable, clear information for your decisions.<\/p>\n\n\n\n<!--more-->\n\n\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter size-large\"><a href=\"https:\/\/surveyrock-blog.s3.amazonaws.com\/blog\/wp-content\/uploads\/2015\/05\/Survey_Design_Checklist.jpg\"><img decoding=\"async\" src=\"https:\/\/surveyrock-blog.s3.amazonaws.com\/blog\/wp-content\/uploads\/2015\/05\/Survey_Design_Checklist.jpg\" alt=\"Survey_Design_Checklist\"\/><\/a><\/figure><\/div>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Define Objectives<\/strong><\/h3>\n\n\n\n<p>Did you write down what decisions will be made based on the data?<\/p>\n\n\n\n<p>Do you have no more than 3 main objectives?<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Outline the Survey<\/strong><\/h3>\n\n\n\n<p>Does your survey include an introduction explaining purpose of the study, duration, and incentive information?<\/p>\n\n\n\n<p>Does the survey begin with a closed-ended question?<\/p>\n\n\n\n<p>Do you have questions at the beginning of the survey to select appropriate survey respondents to continue with the survey?<\/p>\n\n\n\n<p>Do the questions go from general to specific?<\/p>\n\n\n\n<p>Are the demographic questions at the end?<\/p>\n\n\n\n<p>Are any sensitive questions toward the end of the survey?<\/p>\n\n\n\n<p>Do all questions relate to your objectives?<\/p>\n\n\n\n<p>Do you complete each topic before moving on to the next?<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Draft Questions<\/strong><\/h3>\n\n\n\n<p>Do questions avoid use of jargon, acronyms, or terminology unfamiliar to respondents?<\/p>\n\n\n\n<p>Are the questions simple and concise?<\/p>\n\n\n\n<p>Do you ask questions before describing the rating scale?<\/p>\n\n\n\n<p>Are all possible answer options included, or did you include an \u201cOther\u201d option?<\/p>\n\n\n\n<p>Do you allow the respondent to select \u201cPrefer not to answer\u201d for sensitive questions?<\/p>\n\n\n\n<p>Do you display the most positive answer options first?<\/p>\n\n\n\n<p>Do you include a midpoint answer on rating scales?<\/p>\n\n\n\n<p>Are you using unbalanced scales on questions for which you expect the majority of answers to be in a top 2 box?<\/p>\n\n\n\n<p>Are you using closed-ended questions whenever possible for data analysis?<\/p>\n\n\n\n<p>Are open-ended questions voluntary?<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Review and Pre-Test<\/strong><\/h3>\n\n\n\n<p>Have you sent the survey to colleagues or friends to review question wording and survey timing?<\/p>\n\n\n\n<p>Have you pre-tested the survey by first sending to a small group of actual survey respondents to ensure data is being collected as expected?<\/p>\n\n\n\n<p>Does the survey take 10 minutes or less to complete? 5 closed-ended questions take approximately one minute, and 2 open-ended questions take approximately one minute<\/p>\n\n\n\n<p>Will you be able to make the decisions you identified in your objectives based on the data?<\/p>\n\n\n\n<p>You can find additional online survey tips and suggestions <a href=\"https:\/\/www.surveyrock.com\/blog\">on our blog<\/a> to help your survey experience be a success. \u00a0If you haven&#8217;t created your survey yet, <a href=\"https:\/\/www.surveyrock.com\/home\/#chart-option\">sign up for free<\/a> with SurveyRock.<\/p>",
